Guardian Contact

The Guardian Contact feature in TeleDent allows the ability to attach a guardian's contact information to a patient.  

To assign a Guardian to a patient account click on the "Patient" icon mceclip0.png 

Select the patient from the list

At the top of the bar select the "Account" tab

This will open the "Account and Contact Information" page.  

Select the Point of Contact:

  • Patient
  • Guardian
  • Emancipated Minor

Select Guardian, this will ask to select a reason for Guardianship:

  • Age
  • Medical
  • Other

Select the Guardian 

  • If an existing contact in TeleDent chose from the list
  • If the Guardian is not in TeleDent select "Add a New Guardian"

Adding a New Guardian

A screen will pop-up similar to adding a new patient.  You must complete the field to invite the Guardian to the patient portal

  • First Name
  • Last Name
  • Guardian date of birth
  • Invite via email or SMS/Text
  • Valid email and/or mobile phone number

Click "Create"

After adding all relative fields click "Save" to complete.

When the guardian is invited to the portal where they will communicate with the patient's provider, they will create a User Name, Password, enter THEIR birthday, and agree to the Consent Policy and Terms of the Agreement
